Evelyn Sallah-Koomson, FNP-C Family Medicine
Evelyn Sallah-Koomson, FNP-C is a board-certified Family Nurse Practitioner dedicated to helping patients manage chronic conditions, prevent disease, and maintain optimal health and well-being.
Evelyn began her nursing education at King’s College London, where she earned her diploma in General Nursing. She later completed her Bachelor of Science in Nursing at Western Governors University and earned her Master of Science in Nursing (MSN) with a specialization in Family Nurse Practice from Chamberlain University.
With over 19 years of experience in emergency and pediatric emergency care, Evelyn has managed a wide range of high-acuity conditions, including, strokes , myocardial infarctions, abdominal pain, gallbladder attacks, and asthma exacerbations, all requiring prompt and often life-saving interventions. Her extensive clinical background gives her a unique perspective in primary care, allowing her to effectively bridge the gap between the hospital and community setting.
As a primary care provider, Evelyn draws on this vast experience to proactively identify and manage early warning signs, aiming to prevent avoidable emergencies and hospitalizations. She is deeply committed to delivering high-quality, compassionate care and takes pride in building lasting relationships with her patients through education, prevention, and personalized treatment plans.
Evelyn believes in fostering long-term relationships with patients and their families. In her free time, she enjoys traveling and spending time with her family.
